RESUMO

Facial fractures cause postoperative morbidity, including edema, pain, and trismus. Elastic therapeutic tapes are used for optimizing recovery. Background: The aim of the present systematic review and meta-analysis was to evaluate the effectiveness of elastic tape Kinesio taping (KT) in reducing postoperative morbidity in facial fractures surgeries. Methods: A systematic review was conducted in accordance with the PRISMA guidelines. Searches were conducted in the Cochrane, Medline, Scopus, Embase and Web of Science databases using a pre-established search strategy. Results: A total of 811 studies were retrieved after the duplicates were removed, and only randomized clinical trials were included. Eight trials, involving 319 participants, were deemed eligible. One study solely investigated the effect on edema, while the others analyzed at least two of the variables of interest. Results from two RCTs, where qualitative analysis was applicable, suggest a potential reduction in edema in the KT group compared to the control group on the second (RR -0.55, 95% CI -0.89 to -0.22; p = 0.01; I2 = 0%) and third postoperative days (RR -0.71, 95% CI -1.01 to -0.40; p < 0.00001; I2 = 0%). Conclusions: KT is effective in controlling postoperative edema following surgery for facial fractures. However, the effects on pain and trismus should be explored further in studies with standardized methods.

PURPOSE: Surgery of impacted lower third molars may be associated with postoperative complications. The aim of this study is to determine whether piezoelectric surgery is effective in reducing pain, swelling, and trismus compared to conventional rotary instruments during extraction of impacted lower third molars. METHODS: For this systematic review, the searches were performed independently by 2 researchers. Randomized clinical trials that used the piezoelectric instrument for the removal of impacted lower third molars in humans were included. The predictor variable was the study group (piezo surgery vs rotary instruments). The main outcome was the analysis of postoperative pain, swelling, and trismus. Data analysis included risk of bias assessment (RoB 2 Cochrane) and meta-analysis with heterogeneity based on random effects I2 and 95% confidence interval. RESULTS: In the initial results, 956 articles were revised and after applying inclusion/exclusion criteria, the final sample was composed of 18 publications, all being randomized clinical trials. The results of this meta-analysis demonstrated a significant reduction in pain scores with a Cohen's d of -0.95 [CI 95% = -1.23 to -0.67] of high clinical impact (P < .001). In the piezo group there was a significant increase in mouth opening of 4.29 [CI 95% = 2.33 to 6.25] mm (P < .001). Regarding swelling, Tragus-Ang and Go-eye, both showed a significant reduction in the piezo group (P < .001). There was a significant increase of 7.32 [CI 95% = 4.40 to 10.24] minutes in the piezo group (P < .00001), and none of the studies showed a significant risk of bias. CONCLUSIONS: Piezo proved to be effective in reducing pain, swelling, and trismus in third molar surgeries even with longer surgical time, but due to the lack of standardization in primary studies regarding swelling, new, controlled and standardized studies should be carried out with the objective of proving the effectiveness of this therapeutic modality in the reduction of postoperative swelling.

The objective of this systematic review is to evaluate the effectiveness of low-level laser therapy in the treatment of herpes labialis. The searches were carried out independently by 2 researchers and the articles were selected through the electronic databases according to the inclusion and exclusion criteria previously established. Initially, 480 articles were found, of which 7 randomized clinical trials and 1 clinical trial were selected. In total, 928 patients were included. In the meta-analysis, the mean healing time for laser use was significant, showing a mean reduction of 1.37 [CI 95% = 0.92 to 1.82] days for tissue healing (p < 0.0001). In the meta-analysis to evaluate the time for crust formation, there was no significant difference between the groups and no significant reduction in the mean time for crust formation (p = 0.150). Only one of the selected studies had a low risk of bias. The use of low-level laser proved to be effective in the treatment of herpes labialis. However, due to the high risk of bias in the included studies, there is a need to carry out new standardized studies to prove the effectiveness of this therapy.

Introdução: Odontomas são tumores odontogênicos, considerados hamartomas de desenvolvimento que frequentemente interferem na erupção dentária. O presente estudo tem como objetivo relatar o caso clínico de odontoma composto em região posterior da mandíbula, desde o achado radiográfico até o momento cirúrgico e confirmação diagnóstica com exame histopatológico. Relato do caso: trata-se de paciente do sexo feminino, 7 anos, branca, encaminhada da Unidade de Saúde da Família (USF) ao ambulatório do Serviço de Cirurgia e Traumatologia Buco-Maxilo-Facial do Hospital Universitário Oswaldo Cruz (UHOC), devido a um achado radiográfico radiopaco, de limites bem definidos, com halo radiolúcido circundante, no corpo da mandíbula do lado esquerdo, entre as raízes dos dentes 74 e 75. O tratamento proposto foi a excisão cirúrgica conservadora sob anestesia geral. Após a retirada da lesão, foram identificadas calcificações múltiplas diferentes tamanhos e formas, de assemelhando-se a dentículos, encaminhadas para exame histopatológico e confirmando o diagnóstico de odontoma composto. Após 7 dias, observou-se um processo de cicatrização satisfatório. Ela será acompanhada para analisar a progressão do reparo e movimentação da bolsa óssea na dentição permanente na região. Considerações finais: Portanto, recomenda-se o tratamento cirúrgico conservador, através da remoção completa da lesão e preservação máxima dos dentes retidos. Introduction: Odontomas are odontogenic tumors, considered developmental hamartomas that often interfere with tooth eruption. The present study aims to report the clinical case of compound odontoma in the posterior region of the jaw, from the radiographic finding to the surgical moment and diagnostic confirmation with histopathological examination. Case report: this is a female patient, 7 years old, white, referred from the Family Health Unit (FHU) to the outpatient clinic of the Oral and Maxillofacial Surgery and Traumatology Service of the Oswaldo Cruz University Hospital (OCUH), due to a radiopaque radiographic finding, with well-defined limits and a surrounding radiolucent halo, in the body of the jaw on the left side, between the roots of teeth 74 and 75. The proposed treatment was conservative surgical excision under general anesthesia. After removal of the lesion, multiple calcifications of different sizes and shapes were identified, resembling denticles, referred for histopathological examination and confirming the diagnosis of compound odontoma. After 7 days, a satisfactory healing process was observed. She will be followed up to analyze the progression of bone pocket repair and movement in permanent dentition in the region. Final considerations: Therefore, conservative surgical treatment is recommended, through the complete removal of the lesion and maximum preservation of retained teeth. ABSTRACT: This systematic review was carried out to determine if maxillomandibular fixation, used in the treatment of maxillary fractures, interferes with respiratory function, and appraised studies that evaluated pulmonary function using spirometry on patients with and without maxillomandibular fixation. Searches were conducted on the following databases: Medline/Pubmed, Embase, Cochrane, Web of Science, Scopus, and Scielo, in addition to a search of the grey literature and a manual search. Five studies were included in the qualitative analysis, but it was not possible to conduct a quantitative study due to the lack of data. Forced vital capacity, forced expiratory volume in the first second, the ratio of these values, and peak expiratory flow were the parameters studied. In all the studies included in this review, it was possible to observe the lowest respiratory parameter values during maxillomandibular fixation and, after removal, normal respiratory function was restored. Accordingly, it was possible to conclude that maxillo-mandibular fixation temporarily affects respiratory function, during use, but it is restored on removal, therefore, not precluding its use on healthy patients who have good nasal breathing prior to the procedure. However, in patients suffering from some form of respiratory limitation or pulmonary disease, and also mouth breathers, maxillomandibular fixation should not be used so as to avoid more severe respiratory complications, underlining the importance of preoperative pulmonary evaluation.

Os abscessos labiais são entidades pouco relatadas na literatura científica. São infecções potencialmente graves, de origem poli microbiana, podendo causar necrose tecidual e deformidades na região. A microbiota dessas infeções costuma ser conhecida, dispensando, muitas vezes, a cultura e o antibiograma. Antibióticoterapia empírica e drenagem apresentam boa resolutividade. O presente trabalho tem como objetivo relatar um caso de abscesso labial, causado por manipulação de acne. Paciente do sexo masculino, 19 anos de idade, com relato de manipulação de lesão pustulosa na linha de transição muco-cutânea do lábio inferior há 05 dias, evoluindo abscesso labial. O tratamento foi realizado com antibióticoterapia empírica, penicilina de amplo espectro associado com sal potássico de ácido clavulânico e drenagem. The lip abscesses cases are little reported in the scientific literature. They are potentially serious infections, polymicrobial and may cause tissue necrosis and deformities in the region. The microbiota of these infections usually known, often dispensing culture and sensitivity. Antibiotic empiric therapy and drainage have good resoluteness. This study aims to report a case of lip abscess caused by acne handling. A 19-year-old male patient, presenting a pustular lesion on the mucocutaneous transition line in lower lip with 5 days of evolution. The treatment was performed with empirical antibiotic therapy, broad spectrum penicillin associated with clavulanic acid potassium salt and drainage. Background: To compare the analgesic effect of anesthetic infiltration of lidocaine 2% and low-level laser therapy (LLLT) by GaAlAs into tender points of patients with orofacial pain and fibromyalgia (FM). Material and Methods: A randomized clinical trial was performed with adults (N=66) that were allocated into two groups (1:1): Group A received LLLT irradiation by Diode Laser GaAlAs (780nm) with expositions twice a week during six weeks and Group B was treated with anesthetic infiltration of lidocaine 2% without vasoconstrictor once a week for four weeks. The pain assessment included the Visual Analogic Scale (VAS) and tenderness to palpation. Results: No dropout and adverse effect was observed during the study. The pain decreased significantly in each group after the treatment (p=0.0001, β=1.0), even though no statistical difference was found between both treatments (p=0.46, β= 0.82). The presence of tender points decreased after both treatments, with responsively in some types of masticatory muscles (p<0.05) except posterior temporalis muscle. The patients' perception showed that both treatments were effective and a few patients reported that the treatment did not improve welfare. Conclusions: The LLLT by GaAlAs and anesthetic infiltration of lidocaine 2% were equally effective to control orofacial pain in FM individuals (AU)

OBJETIVO: Verificar a prevalência das perdas dentais e sua relação com fatores socioeconômicos em pacientes das Clínicas de Atenção Básica da Faculdade de Odontologia de Pernambuco (FOP). METODOLOGIA: Estudo transversal, tendo como instrumento de pesquisa um formulário composto de identificação, aspectos socioeconômicos e um exame clínico constando dentes ausentes e etiologia precedente. A amostra foi de 169 usuários, com idades entre 18 e 68 anos. Os dados da pesquisa foram analisados por meio das técnicas de estatística inferencial. RESULTADOS: Foi demonstrado que 76,3% dos pacientes tiveram alguma perda dental. Os dentes mais acometidos foram os molares inferiores e os molares superiores com correspondeu aos de 1,58 e 1,09, respectivamente, e a menor média foi dos dentes anteriores inferiores. O percentual de pacientes sem perda foi maior entre aqueles que não tinham filhos (82,5%) e nula entre os que tinham quatro ou mais filhos. A cárie foi a etiologia mais frequente, com 62,1%. CONCLUSÕES: A prevalência da perda dental foi alta, e a etiologia mais frequente foi a cárie dentária. O grupo de dentes mais acometido pela perda dental foi dos molares inferiores, e evidenciou-se associação estatisticamente significante da perda dental com a faixa etária e número de filhos... (AU)


OBJECTIVE: Determine the prevalence of tooth loss, tooth extraction, and its relationship with socioeconomic factors in patients seen in primary care clinics of the Faculty of Dentistry of Pernambuco (FOP). METHODS: A cross-sectional study that used as a research instrument consisting of a form identification, socioeconomic factors, and a clinical, consisting lost teeth and cause of loss was conducted. The sample of 169 users, aged 18 and 68 years, of both sexes. The survey data were imported into SPSS version 5.0 and analyzed using the techniques of inferential statistics. RESULTS: It was shown that 76.3% of the patients studied had some tooth loss. The teeth most affected were lower molars and upper molars with a mean of 1.58 and 1.09, respectively, and the lowest average was lower anterior teeth (0.17). The percentage of patients without loss was highest among patients who had no children (82.5%) and was zero among those who had four or more children. Caries was the most frequent cause, with 62.1%. CONCLUSIONS: The prevalence of tooth loss was high and the most frequent cause was dental caries. Ameloblastomas são tumores dos maxilareslocalmente agressivos com uma alta propensão pararecorrência e acredita-se que surgem a partir de restos dalâmina dental ou epitélio odontogênico. A cirurgia radicalcontinua a ser a terapia de escolha. Relato do caso:Apresentamos um caso onde uma paciente de 69 anos deidade, aproximadamente oito anos após hemi-mandibulectomiapara tratamento de ameloblastoma multicístico, procurou oserviço de cirurgia buco-maxilo-facial do Hospital UniversitárioOswaldo Cruz, em Pernambuco, Brasil, apresentandoaumento de volume intrabucal doloroso, bem delimitado, emedindo cerca de 4 cm na submucosa adjacente à placa detitânio que foi usada para a reconstrução. Uma tomografiacomputadorizada e uma biópsia incisional foram realizadas.A lesão foi diagnosticada como ameloblastoma. Ameloblastomas are locally aggressive jawtumors with a high propensity for recurrence and believed tooriginate from remnants of dental lamina or odontogenicepithelium. Radical surgery still remains the first-choicetherapy. Case Report: Herein we report the case of a 69-year-old patient admitted to the University Hospital OswaldoCruz in Pernambuco, Brazil, exactly eight years afterundergoing hemi-mandibulectomy for treatment of a multicysticameloblastoma. The patient complained of painful, welldelimitedswelling measuring about 4 cm in submucosa nextto a titanium bone plate that was used for reconstruction.Computed tomographic imaging and incisional biopsy wereperformed. The lesion was diagnosed as a ameloblastoma.The tumor was resected and histopathological analysesshowed predominance of follicular and acanthomatousameloblastoma within a highly fibrotic area, thus completingthe diagnosis of soft-tissue recurrence. OBJECTIVE: To compare changes related to self-esteem and appearance satisfaction between pre and postsurgical phases in patients undergoing orthognathic surgery and to assess the quality of life and psychosocial changes of these patients six months after surgery. METHODS: A longitudinal observational qualitative study was performed. The sample comprised 15 patients with dentofacial deformities who underwent orthognathic surgery. One questionnaire and two forms were answered during pre and postoperative phases. RESULTS: The results showed that 13.3% of patients demonstrated self-esteem improvement, especially in relation to appearance satisfaction. Improvements were also noted in social, occupational and family relationships. With regard to the assessment of quality of life, according to the World Health Organization questionnaire, the lowest improvement averages corresponded to environmental control. CONCLUSION: Orthognathic surgery brings along many emotional changes that should be considered before and after surgery, since the patients' psychological state may be favorable and/or unfavorable during recovery, influencing their quality of life, self-esteem and appearance satisfaction.

OBJETIVO: comparar as mudanças relacionadas à autoestima e satisfação com aparência, entre o pré-operatório e o pós-operatório, de pacientes submetidos à cirurgia ortognática, e avaliar a qualidade de vida e mudanças psicossociais desses pacientes seis meses após a cirurgia. MÉTODOS: estudo qualitativo, de caráter observacional do tipo longitudinal, com amostra de 15 pacientes que apresentavam deformidades dentofaciais e que foram submetidos à cirurgia ortognática. Foram aplicados um questionário e dois formulários, em duas fases: uma pré-cirúrgica e outra pós-cirúrgica. RESULTADOS: os resultados mostraram que 13,3% dos pacientes obtiveram melhora em relação à autoestima e, principalmente, em relação à satisfação com a aparência. Foi também constatada melhoria nas relações sociais, profissionais e familiares. Em relação à avaliação da qualidade de vida segundo o questionário da Organização Mundial da Saúde, observou-se que a média mais baixa correspondeu ao domínio meio ambiente. CONCLUSÕES: a cirurgia ortognática traz consigo uma carga emocional elevada, que deve ser considerada antes e após o ato cirúrgico, pois o estado psicológico do paciente repercute em fatores favoráveis ou desfavoráveis durante a sua recuperação, influenciando de diferentes formas na qualidade de vida, autoestima e satisfação com a aparência.

Aneurysmal bone cyst (ABC) is an uncommon osteolytic lesion of the bones, usually affecting the long bones and spine. The lesion is rare in the jaws and is found most commonly in the body and ramus of the mandible. In some cases, ABCs may be present as rdestructive lesions simulating malignancies. In these cases, a careful diagnosis should be done, including in the differential diagnosis malignant tumors such as telangiectasic osteosarcoma and intraosseous fibrosarcoma. Removing the lesion is usually easy if it is confined within the bone, but it may prove difficult if the lesions are multilocular, expansive, divided by multiple bony septa, or destructive or when the cortical is perforated. Therefore, the surgical treatment of the destructive ABCs should be more radical.

Objective: This paper investigates the relationship between preoperative findings and short-term outcome in third molar surgery. Study design: A prospective study was carried out involving 80 patients who required 160 surgical extractions of impacted mandibular third molars between January 2009 and December 2010. All extractions were performed under local anesthesia by the same dental surgeon. Swelling and maximal inter-incisor distance were measured at 48 h and on the 7th day postoperatively. Mean visual analogue pain scores were determined at four different time periods. Results: One-hundred eight (67.5%) of the 160 extractions were performed on male subjects and 52 (32.5%) were performed on female subjects. Median age was 22.46 years. The amount of facial swelling varied depending on gender and operating time. Trismus varied depending on gender, operating time and tooth sectioning. The influence of age, gender and operating time varied depending on the pain evaluation period (p < 0.05). Conclusions: Short-term outcomes of third molar operations (swelling, trismus and pain) differ depending on the patients' characteristics (age, gender and body mass index). Moreover, surgery characteristics such as operating time and tooth sectioning were also associated with postoperative variables (AU)

OBJECTIVE: This paper investigates the relationship between preoperative findings and short-term outcome in third molar surgery. STUDY DESIGN: A prospective study was carried out involving 80 patients who required 160 surgical extractions of impacted mandibular third molars between January 2009 and December 2010. OBJECTIVE: it was to evaluate intra- and interexaminer reliability and reproducibility of linear measurements in cone-beam tomography (CBCT) images. MATERIALS AND METHODS: CBCT scans were obtained from 50 patients of a private clinic. Two examiners located the landmarks twice in two-dimensional and three-dimensional images on different days, with images performed 1 week apart. Intra- and interexaminer reproducibility and reliability were analyzed. RESULTS: the measurement error (ME) did not show significant differences between intra- and interexaminers. The intraclass correlations (ICC) between the intra- and interexaminer readings obtained with the different methods showed almost perfect matches. The results demonstrated high examiner reproducibility for linear and volumetric parameters with high intraclass correlation coefficient (ICC) and coefficient of variation (CV). The ICC showed that the methodology was highly reliable and reproducible (ICC- >0.99 and CV <1.5%). CONCLUSION: the measurements demonstrated strong agreement between examiners and significant reliability and reproducibility. Therefore, this methodology can serve as a standard for linear measurement analysis of the topography of mandibular canal and osseous adjacent structures.

OBJECTIVE: The objective of this research was to study the influence of the use of helmet in facial trauma victims of motorcycle accidents with moderate traumatic brain injury. METHODS: We retrospectively reviewed the incidence of facial injuries in helmeted and nonhelmeted victims with moderate traumatic brain injury at a referral trauma hospital. RESULTS: The sample consisted of 272 patients predominantly men (94.5%) and between 21 and 40 years old (62.9%). The majority of patients were using helmet (80.1%). The occurrence of facial fractures was most frequent for zygomatic bone (51.8%), followed by mandible (18.8%) and nasal bones (9.2%). CONCLUSIONS: Individuals in the most productive age group are most affected, which causes a great loss to financial and labor systems. It is important to take measures to alert the public regarding the severity of injuries likely to occur in motorcycle-related accidents and ways to prevent them.

O objetivo deste estudo foi analisar as estimativas de incidência de câncer de cavidade bucal no Brasil e em Sergipe, publicadas pelo INCA entre 2000 e 2010. Foram verificados os números de casos e as taxas brutas de incidência previstas para o Brasil, Sergipe e Aracaju, comparando-as com as dos outros Estados e Capitais brasileiros. Também foi observada a incidência estimada para outras localizações anatômicas mais acometidas pelo câncer. No Brasil, entre 2000 e 2010, a cavidade bucal foi uma das dez localizações anatômicas com maior número de casos novos de câncer estimados, sempre ocupando a sétima posição. Em Sergipe, houve um aumento nas estimativas de incidência alterando de 80 para 160 novos casos, com preponderância masculina (80% a 61,5%). Dentre os tipos de câncer mais incidentes, exceto pele não melanoma, a cavidade oral passou a ser, a partir de 2005, uma das cinco localizações com maior número de casos estimados, ultrapassando outros sítios como cólon e reto, esôfago, estômago, melanoma ou leucemia isoladamente. O número de casos novos de câncer de cavidade bucal estimados vem aumentando a cada ano, no Brasil e em Sergipe. Ainda há maior número de casos entre os homens, mas a proporção homem:mulher está diminuindo.


The aim of this study was to analyze the estimated incidence of oral cancer in Brazil and Sergipe published by INCA between 2000 and 2010. It was verified the numbers of cases and crude incidence rates estimated for Brazil, Sergipe, and Aracaju comparing them with those of other Brazilian states and capitals. It was also observed the estimated incidence for other anatomic sites most affected by cancer. In Brazil, between 2000 and 2010, the oral cavity was one of the ten anatomic sites with the highest number of new cancer cases estimated always occupying the seventh position. In Sergipe there was a continuous increase in incidence estimates for 80 out of 160 new cases, with male preponderance (80% to 61.5%). Among the most incident cancers, except non-melanoma skin, oral cavity became, from 2005, one of five locations with the highest number of estimated cases, surpassing other sites such as colon and rectum, esophagus, stomach, melanoma or leukemia alone. The number of new cases of oral cancer estimated has increase each year in Brazil and Sergipe. There are still more cases among men, but the man: woman ratio is decreasing.
